Three Year Old Gaffney Girl Dies in Tragic Daycare Accident

Funeral services were held on May 16, 2011 for 3 year old Alyssa Jade Mayfield of Gaffney, SC.
Authorities state that the toddler died after a 250 lb wooden stage section fell from a cart on top of her.
The accident happened in the gym at West End Baptist Church at approximately 3:20 pm.
Witness' stated that the little girl was retrieving a ball when she bumped into the wheeled cart that was being used to store the wooden stage pieces. Daycare workers performed CPR on the little girl until she was airlifted to a Spartanburg Hospital where she was pronounced dead about an hour later.
The church pastor released a statement that the four stage sections were kept on a wheeled cart that had a metal strap meant to secure the sections to the cart and prevent them from falling.
According to a press release by Cherokee County Coroner Dennis Flower, the official cause of death was blunt force head trauma.
DSS said that 4 violations were found which date back to 2009. The agency is conducting an investigation and will release it's findings within a few weeks.
